This reevaluation request form is used to request a review of an established transfer course to potentially satisfy a Baccalaureate Core category.  This review requires the submission of additional supporting documentation, typically a course syllabus. Once submitted, the Office of the Registrar will review the request and attached supporting documents. They will communicate the outcome within 10 business days.

Please note:  

• This form is only to be used for transfer coursework. Requests cannot be accepted for OSU coursework. Requests for initial or reevaluation of OSU GO/Study Abroad coursework should be sent to [email protected]. 
• Only one Baccalaureate Core category may be submitted per form. You will need to complete additional forms if your are requesting other reevaluation for other categories.
• Transfer courses may only carry and fulfill one Bacc Core category.
• Only one transfer course may be submitted per form. You will need to complete additional forms if your are requesting reevaluations of multiple courses.
• This form is not to be used as a request for individual exception to a decision that has already been made. This is an opportunity for students to submit additional materials for review where the information available during initial review may not have supported applying a Bacc Core category on its own.
• This form cannot be used for transfer coursework that has been determined to be a direct articulation (i.e. "equivalent") to an OSU course. Transfer courses that come in as direct articulations must carry, or not carry, the same Bacc Core category as the OSU course.
• To request a transfer course to satisfy the Writing Intensive Curriculum (WIC), please meet with your academic advisor to work through the WIC Transfer Course Checklist.
• Requests to review a transfer course for direct articulation (i.e. "equivalency") must be submitted via the Transfer Course Petition for Direct Articulation or a college-specific form, if applicable. Please work with your Academic Advisor to determine the most appropriate form. 

You will need to attach the following documents to this request form:         

• Transfer course syllabus that includes a week-by-week summary of lectures, readings, assignments, projects, exams, etc.
• Optional supplemental documents that support your petition request.

Contact and Submission Information:

The Office of the Registrar, with the assistance of academic units and the Core Education Committee, determines the transferability of coursework completed at other institutions.  Initial evaluations are determined for articulation to OSU courses and completion of Baccalaureate Core category requirements.  This transfer course petition provides a secondary opportunity to evaluate the initial articulation for a transfer course with additional supporting documentation.  

Request Approval Process        

1. The student will submit the completed "Transfer Course Reevaluation Request for Baccalaureate Core," accompanied by any supplemental documents for review.  
2. The Office of the Registrar will conduct an evaluation within 10 business days of receiving the petition.
3. Final decisions will be communicated to the student through OSU email.
